Staff member involved in the incident. Select the appropriate staff code (e.g., "MLS") – the name will auto-fill beside it. |

The date and time the incident occurred. Use the calendar picker or enter manually. |
Logged By |
Automatically displays the system user who is currently logged in and recording the entry. |
Date Logged |
Timestamp of when the incident record was entered into the system. This is automatically filled in. |
Incident ID |
Reference ID for the incident type, pulled from the master incident table. |
Category |
General category of the incident (e.g., Health & Safety, Conduct, etc.). |
Type |
Specific type within the category (e.g., Workplace Injury). |
Description |
A short summary of the incident. |
Suggested Action |
Recommended next steps or procedural follow-up (e.g., "Incident report, ACC claim"). |
Incident Notes |
Free-text field for detailed notes describing what happened, actions taken, and any relevant follow-up. This is important for compliance and future auditing. |
