This screen is used to record any health, safety, or HR-related incidents involving staff members. Accurate and timely logging of incidents ensures compliance with workplace safety regulations and supports follow-up actions to prevent recurrence.

Description

Incident ID (Top Left)

Auto-generated unique identifier for the incident log entry.

Incident Happened To

Staff member involved in the incident. Select the appropriate staff code (e.g., "MLS") – the name will auto-fill beside it.

Date of Incident

The date and time the incident occurred. Use the calendar picker or enter manually.

Logged By

Automatically displays the system user who is currently logged in and recording the entry.

Date Logged

Timestamp of when the incident record was entered into the system. This is automatically filled in.

Incident ID

Reference ID for the incident type, pulled from the master incident table.

Category

General category of the incident (e.g., Health & Safety, Conduct, etc.).

Type

Specific type within the category (e.g., Workplace Injury).

Description

A short summary of the incident.

Suggested Action

Recommended next steps or procedural follow-up (e.g., "Incident report, ACC claim").

Incident Notes

Free-text field for detailed notes describing what happened, actions taken, and any relevant follow-up. This is important for compliance and future auditing.

Use the Suggested Action field to assign follow-up tasks or documentation (e.g., ACC paperwork).
